Providence is calling a Clinical Pharmacist - Critical Access Hospital & Long Term Care (Acute Pharmacy, Days, 1.0 FTE) to Providence Seward Medical Center in Seward, Alaska. In this position you will:

  • Develop and maintain policy and procedures for the Pharmacy or Licensed Drug Room to be in compliance with federal, state and local regulations. Creates, maintains, and follows department policies and procedures to ensure safe, accurate, and timely provision of all medications. Plans, assesses, implements, evaluates, and documents all aspects of pharmaceutical care. Supports the policies and goals of the medical center and long-term care facility to ensure the effective delivery of pharmaceutical care and that caregivers are able to perform their work efficiently.

  • Responsible for the delivery of clinical pharmacy services and medication oversight for the medical center and long-term-care center. Reviews medication orders for appropriateness and monitors drug therapy for safety, efficacy, and desired clinical outcome. Communicates relevant findings and documents recommendations to other health care practitioners in the medical record as appropriate. Screens for drug-drug and drug-food interaction, adverse drug reactions, drug allergies, and concomitant drug use. Recommends initial or alternative drug therapies. Evaluates laboratory and other clinical data available in the patient medical record as it pertains to drug therapy. Provides verbal or written consultations in the areas of drug therapy selection and pharmacokinetics dosing. Participates in the management of medical emergencies, adverse drug reactions, and acute and chronic disease states.

  • Responsible for safe, timely, and effective drug distribution services. Compounds, labels, and packages medications and pharmaceuticals meeting professional standards and guidelines. Provides IV admixtures as needed in accordance with USP-797 standards. Insures controlled substances are stored securely, distributed safely, and conducts audits to detect and prevent drug diversion. Conducts routine audits of medication storage areas to insurer all medications are in date and are stored securely.

  • Responsible for managing pharmaceutical purchasing for the medical center and long-term care center. Collaborates with the region pharmacy buyer and other ministry buyers to insure optimal purchasing of medications and limit medication waste. Responsible for managing 340b program participation, including routine audits of 340B medication use to insure full compliance with the 340B program. Collaborates with System and Region 340B resources to optimize participation in the 340B program.

  • Responsible for providing information and education regarding drug therapy to patients and other health professionals. Responds to drug information requests from physicians, nurses and patients. Provides drug information and educational programs to the medical staff, nursing staff, other health care providers and patients. When requested, assists nursing in discharge planning and patient education related to the appropriate use of medications.

  • Participate in Quality Improvement activities for the medical and long-term care center. Conducts drug use reviews, chart reviews, and audits to identify medication use improvement opportunities and to insure compliance with medication management standards of the Joint Commission, CMS, Board of Pharmacy, and other applicable regulatory entities. Supports the local Pharmacy and Therapeutics Committee and serves as a liaison between the local and System Pharmacy and Therapeutics Comm. formulary decisions. Educates providers and staff regarding P&T decisions and helps to optimize formulary compliance. Participates in Region and System Pharmacy quality, safety, utilization management, drug formulary, purchasing, and cost-saving initiatives.

  • Support the local Pharmacy and Therapeutics (P&T) Committee and serves as a liaison between the local and System Pharmacy and Therapeutics Comm. formulary decisions. Educates providers and staff regarding P&T decisions and helps to optimize formulary compliance.

  • Participate in the regional pharmacy operations council and serves as a liaison between the medical and long-term care center and region pharmacy services to insure optimal pharmaceutical care is provided. Assists in the development of the pharmaceutical budget and monitors budget performance to identify revenue and expense variances, and risks associated with changes in the pharmaceutical industry market.

  • Orient new staff to the pharmacy/drug room and provides continuing education in providing pharmacy services to ensure competency of staff. Provides supervision and guidance and to staff members, which contribute to the overall quality of pharmacy services. Maintains and enhances professional growth and development through participation in educational programs, current literature, in-service meetings, and workshops; demonstrates a comprehensive clinical pharmacy knowledge base.

  • Initiate and effectively utilizes problem solving skills to promote a positive work environment and interpersonal relationships, while maintaining professionalism and confidentiality.

  • Implemen care/services that recognize age/diversity specific needs/issues of customers served.

  • Complete Competency Plan for assigned job and department functions.

Qualifications:

Required qualifications for this position include:

  • Bachelor's Degree in Pharmacy

  • Current Alaska Pharmacist License

  • BLS or the ability to attain certification within 90 days of date of hire

  • One (1) year experience in an Acute Care or Long Term Care setting.

Preferred qualifications for this position include:

  • PharmD

  • Three (3) years experience in an applicable setting.

Serving Seward and the tourism/cruise ship industry, Providence Seward Medical Center is a collaborative effort between an Alaskan community and Providence Health System. In 1996, the City of Seward awarded Providence a long-term contract to manage and operate its municipally owned hospital and clinic. In addition to 24-hour emergency room services, the hospital provides inpatient care for needs that can be handled by the Seward primary care physician and physician assistant team, outpatient clinic services, physical therapy, diagnostic and home health care services.
